Notice what helps your puppy settle. Record care needs and handoffs—not a score or a prescribed sleep target.

Use one row for a rest opportunity or waking event. Record only what you observed. Leave unknown details blank; do not estimate a duration from memory.

Example (illustrative): “Bed beside caregiver; woke and needed toilet care; returned to the same bed and settled.”
